Description: Htg13-07293 i had originally submitted completion card when the project was completed. then in october of this year i received a letter with a fine stating that there was no completion card received. i left several messages at numbers such as 317-327-8408 and 317-327-7800 trying to resolve the issue. another letter arrived in november with a higher fine. again left messages, never received a return call. finally was able to reach someone thru email to learn what i needed to do. i know the rules. i'm not sure why this happened. ***see activities
