Description: ****the permit completion date has been extended through november 5 2010 on 10/22/2010 per k. scott**. **the permit completion date has been extended through july 31 2010 on 6/15/2010 per k. scott**. by accepting this permit permittee acknowledges the following: a) that time is of the essence; b) they have satisfied themselves that the owner has provided satisfactory r/w and/or easement(s) in accordance with city code to accommodate the work; c) to give city & adjacent residents 48 hrs advance notice prior to starting work; d) to construct the improvements as indicated on the permit plan record documents under plan file no. "crbl 201010858" and contract no. "1" with work not starting prior to april 13 2010; (city agrees to early issuance of this permit subject to applicant agreeing to pay a triple permit fee if work is started before the start date indicated herein); e) to complete this work by the permit completion date of may 12 2010; f) to coordinate with the work of all other contractors for timely completion; g) to remove/relocate/reconstruct unacceptable work to the satisfaction of the city; h) to remain responsible for the work until permit and bond are released; i) the city will notify the permittee by letter when the maintenance period starts if applicable under the provisions of the bond for this permit. maintenance shall include any and all repairs to damaged components of the facility that may have occurred during the maintenance period; j) the city will notify the permittee by letter when this permit and the facility constructed under it are final accepted. final acceptance shall not occur until the maintenance period required under the bond has been completed and all maintenance items addressed. k) they have satisfied themselves that subgrade conditions are satisfactory and suitable for installation of the improvements proposed under the scope of this permit.

Description: *the permit completion date has been extended through april 30 2011 on 11/5/2010 per k. scott*. bond req'd yes x no. by accepting this permit permittee acknowledges the following: a) that time is of the essence; b) they have satisfied themselves that the owner has provided satisfactory r/w and/or easement per city code to accomodate the work; c) to limit disturbed area to the permitted acreage of 9.40 acres. d) to give city & adjacent residents 48 hrs advance notice prior to starting work; e) to install and maintain bmp's in accordance with ch. 63 code of ordinance and as indicated on the permit plan record documents under the file number 2010-002a rev. 1 and contract number 1 indicated hereon; f) to complete this work by the permit completion date shown hereon; g) to coordinate with the work of all other contractors for timely completion; h) to remove/relocate/reconstruct unacceptable work to the satisfaction of the city; i) to remain responsible for the work until permit and/or bond are released. this permit does not authorize grading or excavation in excess of 6 in. above or below existing grade except for the removal of tree stumps. failure to obtain any required grading permits for earthwork in excess of 6 in. may result in enforcement actions as provided under the city code.
